UPSC CMS DAF Form 2019 Out At upsconline.nic.in; Documents Needed

The Detailed Application Form-1 (DAF) has been released by the Union Public Service Commission (UPSC) for Competitive Examination for the Combined Medical Services on the official website. Those candidates who have cleared the UPSC CMS computer-based test can visit the official website i.e., https://upsconline.nic.in/ and submit their application forms.

The candidates can submit their UPSC CMS exam forms till 6 pm on 9 September 2019 only.

UPSC CMS DAF: Documents Required

The candidates would require scanned copies of the following documents to submit their UPSC CMS Detailed Application Forms:

  • Age certificate
  • Educational qualification certificates
  • Certificate of Scheduled Castes/ Scheduled Tribes/ Other Backward Classes/ EWS/ Physically Handicapped, if applicable
  • OBC certificate, if applicable
  • Duly signed and filled proforma provided as Annexure to DAF by the candidate seeking relaxation as OBC candidate
  • Certificate for age relaxation, if applicable
  • Certificate supporting the claim of being Persons with Benchmark Disability, if applicable
  • Duly signed copy of the undertaking if the candidate is already in government service

The official notification for filling the UPSC CMS DAF states, “Duly signed copy of the online Detailed Application Form along with originals of the above certificates are required to be produced along with self certified copies of all the documents at the time of interview or within the last day of interview, which will be published in the Commission’s website in due course, failing which their candidature is liable to be cancelled. It may also be noted that in no case the last date of verification of certificates will be extended. Candidates must also note that they should upload only the scanned copies of the original documents and no document is required to be sent by post.”
